Documents Description
Floodway Boundary & Floodway Maps FBFMs) Scanned copies of all the City of Flagstaff FBFMs. These maps show the floodway boundaries. They do not show areas revised by LOMRs (see the FIRMs for the locations of LOMRs). An index map is included. (15 documents: 5.60 MB)
Flood Insurance Rate Maps FIRMs Scanned copies of all the City of Flagstaff FIRMs. These maps show the floodplains within the city. The maps have been annotated to show where they have been revised by LOMRs. An index map is included. (17 documents 8.83 MB)
Flood Insurance Study Flood Insurance Study, City of Flagstaff Arizona Revised: August 2, 1996. This is a two part document. Part one is the text and Part two are the profiles.
Letters of Map Revision (LOMRs) Copies of all LOMRs affecting the City of Flagstaff. The file name indicates the date of the LOMR(e.g. “LOMR 1-4-96”). Maps that show the revised floodplain and floodway limits can be found at the end of each LOMR document. (18 documents: 10.0 MB)
Digital Flood Insurance Rate Maps (DFIRMs) Several of the recent LOMRs within the city are shown only on draft copies of the Federal Emergency Management Agency’s new DFIRMs. Only those DFIRMs that include recent LOMRs are provided here. The DFIRMs will eventually replace all the city’s existing FIRMs and FBFMs. A complete set of the DFIRMs will be included here once the maps have been officially adopted. (24 documents: 198 MB)
Rural Floodplains Some floodplains within the city have been designated as “Rural Floodplains.” This document shows these floodplains and describes the restrictions for developing in these areas. (2 documents: 744 KB)

Documents Description
Floodplain Ordinance City Ordinance No. 2001-06: Floodplain Management Regulations. These regulations describe the restrictions, requirements and procedures that must be met for any development within a flood hazard area. (1 document: 228 KB)
Floodplain Use Permit Application Form Per Ordinance 2001-06, a Floodplain Use Permit is required for most development activities within a flood hazard area. This is the application form needed to obtain a Floodplain Use Permit. (1 document: 228 KB)
Elevation Certificate Form **(NEW) April 2009** An elevation certificate is required for all new structures in a flood hazard area and is usually required to obtain flood insurance. A registered professional surveyor, engineer or architect must complete this form. As of April 2009, both the Elevation Certificate & the Floodproofing Certificate can be found in 1 pdf document. By using this download you will have both the Elevation Certificate and the Floodproofing Certificate.(1 document: 507 KB)
Floodproofing Certificate Form A floodproofing certificate is required when a non-residential structure that is located in a flood hazard area has been floodproofed in lieu of being elevated. A registered professional engineer or architect must complete this form. As of April 2009, the Floodproofing Certificate & the Elevation Certificate are in one pdf document. Substantial Improvements Information Sheet This document describes limitations on making “substantial improvements” or “substantial repairs” to structures located within a flood hazard area that do not meet the requirements described in Ordinance 2001-06. (1 document: 160 KB)
Elevation Datum Conversion Information This document provides information that can be used to convert elevations given in the National Geodetic Vertical Datum of 1929 (NGVD 29) to the North American Vertical Datum of 1988 (NAVD 88). Floodwater elevations shown on the LOMRs and FBFMs are NGVD 29. Floodwater elevations shown on the DFIRMs are NAVD 88. (1 document: 47 KB)
